Premium VPN services: the short answer
The large premium VPNs charge more because they maintain things that cost real money: their own apps on every platform, thousands of servers in dozens of countries, regular third party audits, round the clock support and heavy advertising. Some of that benefits you directly. Some of it, above all the advertising budget, you simply help to pay for. Whether premium VPN services are worth it depends on which of those items you would actually use.
If you need forty countries and an audit report to show your employer, pay for a premium brand. If you need one tunnel that works on filtered networks and a fair price, you are paying for features you will never touch.

Packaging versus substance
Bundled extras make a plan look like more for your money. A password manager or an antivirus added to a VPN is seldom the best tool in its class, and it ties more of your digital life to a single subscription. Server counts are easy to inflate with virtual locations. Claims about speed depend on where the test was run. The items with substance are the dull ones: audits, ownership, jurisdiction and a record of how the company behaved when it was tested by a court or a breach.
When comparing any VPN services provider, ask for the date of the latest audit and ask what it covered. An audit of a browser extension says nothing about the servers.
